Creating a Form (4)

The above screen is giving you the options to select various formats for the form and it is preset to Columnar. That will do just fine for our form right now. You can punch some of the other options just to see what they look like by clicking your mouse inside the little circles, but return to the Columnar option before punching the next button. When you are ready, punch the Next button.

In the above form, type AddressEntryForm for the name. When the form first comes up some other name will be there but you can type in a new name. The name Access has already put there will disappear as soon as you start typing.
